Provide the required information for registering an apprenticeship program for Federal purposes with the Department of Labor. Prospective program sponsors must fill out all required fields and may work with the appropriate Registration Agency (either OA or the relevant State Apprenticeship Agency) to better understand what information must be included for each field. This form includes a Definitions/Instructions section which spells out detailed instructions for each field.

Provide the required information for registering an apprentice in a registered apprenticeship program. The form includes sections for information about individual apprentices, details of the registered apprenticeship program (including key information for apprentice awareness of the aspects of a registered apprenticeship program, such as the term length of the program, the apprentice wage schedule, and information about the related instruction aspect of the program), and a final section for information provided by the Registration Agency. This form includes a Definitions/Instructions section which spells out detailed instructions for each field. 

PDF FormatETA Form 671 Voluntary Disability Disclosure

Voluntarily self-report a disability (or a previous disability) to the sponsor of a registered apprenticeship program. This information supports programs’ efforts to provide equal employment opportunity in apprenticeship and ensure that anyone can participate in a registered apprenticeship program. Program sponsors and the Registration Agency will maintain confidentiality about all aspects of apprentices’ disability status. 

PDF FormatETA Form 9039

For apprentices to file a complaint alleging discrimination or other mistreatment during the course of participation in a registered apprenticeship program. Copies of the complaint form are sent to the program sponsor that is the subject of the complaint, as well as the Registration Agency. OA’s regulations require that sponsors of registered apprenticeship programs (and any participating employers) must take necessary steps to assure there is no retaliation against any person who files a complaint, and the contents of the complaint will be subject to confidentiality protections. To understand the updates made to these forms during their most recent reauthorization (approved by the Office of Management on Budget on June 14, 2021), see Bulletin 2021-88, which was issued on August 26, 2021. OA’s updates modernized and streamlined these forms so that only information relevant to the registration of apprenticeship programs and apprentices is collected and made minor clerical edits to improve clarity. The Bulletin summarizes adjustments made to Form 671, Sections I and II, and the ETA Form 9039 Compliant Form, and imbeds attachments of the updated versions of the forms.
